Most Important Instruction for processing Payment Please Do not Miss After Payment, it will reflect an option named SEND ANYWAY. You are requested to Click It. Kindly read the instructions given below very carefully: 1) Minimum Age: 3 Years as on 31st March 2025.(The child born between 1.3.2021 to 30.4.2022) 2) Fill the form in Capital letters only. 3) Online Registration will be done through the school website – www.davashokvihar.in 4) After registering your Mobile No, please wait for 5 minutes to receive the password. 5) Payment will be made ONLINE. 6) If the payment process is not completed successfully due to any reason, proceed again using the same Mobile No & password./ Contact School 7) Keep your details of Debit Card/Credit Card/Net banking ready for online payment of Rs. 25/- as the registration amount 8) In the case of a single parent, the entire information of the parent is to be repeated in both columns. 9) Fill up the form completely before taking a printout. In case of incomplete form/documents, the application will be deemed as rejected. 10) Print out of the filled registration form, duly signed by the parent, must be dropped in the box placed at the School gate, along with self-attested photocopies of the supporting documents. 11) The Last date of online submission of the Registration form for pre-school is 20-12-2024 12) The Last date to drop the Registration form along with the required documents at the school counter is 20-12-2024. 13) Kindly keep the photographs in SOFT copy ready for uploading while filling the form. (Applicant’s, Father’s and Mother’s photograph). The size of the photograph should not exceed 200 KB and should not be less than 50KB. . 14) Transport facility is available on existing routes and stops. Required Documents : (i) Copy of Birth certificate of the child issued by MCD/NDMC/Municipal Corporation. (ii) Proof of Address : • Ration card/Smart Card issued in the name of parents (having name of child). • Domicile certificate of child or his/ her parents. • Voter I-card(EPIC) of any of the parents. • Electricity bill/MTNL telephone bill/water bill in the name of any of the parents. • Aadhar Card/UID Card issued in the name of any of the parents Please note: Mobile Phone and internet bills will not be acceptable. Acknowledgment receipt of Aadhar card/UID card will not be acceptable. (iii) Proof of alumni of D.A.V. PUBLIC SCHOOL (If Applicable): Class XII (Passing Certificate in the name of Mother/Father). (iv) Proof of Sibling (If Applicable): Latest Fee Bill / Report Card / Identity Card
